支援者の方々にお聞きしたいことがあります。

今月のリクエスト募集にてGoogleフォームを利用してみたのですが、使い勝手や改善点などを聞かせて欲しいです。

（I tried using Google Forms for this month's request collection, and I would like to hear your thoughts on usability and improvements.）

現在、予想を下回るほどにリクエストが送られていないことに気付き、何か使いづらい面などがあるのでは？…と心配しております。
