Yes, Ren feels responsible for Kazuma's sickness. Ren probably feels like Kazuma got sick waiting in the rain for him, but also the deed they did the day before, AND Kazuma crying after. Ren feels that he took advantage of his friend. I am sure the guilt is immense, and I wonder if that is why Ren is avoiding emotional attachment and opted for FWB. That, and the dumbass thinks Kazuma is straight. You mentioned why Kazuma was crying. This might be a typical reaction after having gay sex the first time, and Ren likely believes that and he broke his friend. But the narration told us exactly what Kazuma was thinking. Kazuma was only thinking about how Ren's first time should have been better. Kazuma worried it was not a good enough location, and probably thought he himself was not a worthy partner. Kazuma clearly has Ren up on a pedestal. He is "Dazzling" to him. How Ren does not see this is a testament to either Kazuma keeping his emotions in check, or that Ren's awareness of others is just as broke as his gaydar.

They are doing a good job with adaptation from the manga, especially with the scene of their first time as kids they made it very delicate by responding to the manga that it seemed unsuitable for 14-year-old boys to me….The scene of the first time broke me both in the manga and in the series
