This episode gives you another insite, into the Salvatore family and the dinamic between the brothers. Their parents were truly shitty parents. Their father was horrible, to them and to his wife, and ther mother was selfish and as soosn as she got the chance to leg it, she did and left her children behing to deal with their father, she left a seveteen years old Damon to deal with his father alone and to protect his own little brother from him. He had to act like a parent. That is why they were so close, and that is why they still are. When their mother died, Damon was strucked by grief and by the burden of the responsibility he had to take with his little brother. And i you think back, Katherine was his way to have a life on his own, a life with someone that loved him. That is also why he was never angry with Stefan for slepping with Katherine too, why he was pissed of with Stefan for turn him, but really never act on that, why he left Stefan leaving his life te way he wanted, away from him during the war time, because he never burden Stefan with any of his traumas, why even though he loved Elena deeply he was the one patching them up when needed, because Stefan is his little brother but he was also his parent too. That is why Stefan has this codependency to Damon, why his life without him is unbearable, why Stefan loves him above anybody else, because that is his big brother but also the only real loving parent he had, that protected him from their father, so that he could have a fairly normal life and be the shelthered golden boy unware of what was going on in their family and never experience the brutality of life as Damon did, by going to war to please and keep their father happy. Oh yes, Damon has an amazing heart, that is why we love this character so much.

The hurt on Damon's face, when she told him she was not returning to them, tell us everything about Damon. The two women he knew and loved deeply, fail him, crushed him. Stefan was too young to realise what was going on in that house hold but Damon took the full blow of it. That is why he needs time too think about the cure, because Elena is the only person that loved him sincerely, the only person that showed him kindness and compassion, that understands him, and he is not prepared for all the implication that the cure would mean if Elena would want to take it. He needs some serious thinking over it, some processing, especially in the light of the demolition of the image he had in his mind that their mother cared for them, that he discovered was another disillusionment, and bitter disappointment.

Remember in season 3, episode 1912, when Damon and Stefan had not seen each other for 48 years? We could see the love those brothers had for each other despite their disagreement. Damon and Stefan didn't stop loving each other during all those years they didn't see each other. Now, a mother doesn't stop loving her children, especially if they have not done anything to her, so Lily’s issue goes behind what she told Damon in this episode.
