There's three general notions on how Ghouls turn. Either its a specific radiation exposure that isn't quite enough to kill them but instead mutates their cells, or its a genetic lottery (which is possible since nuclear power was used almost everywhere for over a hundred years before the Great War - plenty of time for gradual exposure to mutate a certain percentage of each new generation) or its caused by overdosing on anti-radiation medicine while suffering extreme poisoning. Or maybe a bit of all three - since in the games there's a couple of pre-war Ghouls still around and even a few Ghoul children.
