Brief summary of my process, I uploaded the .psd file so you can see all the layers and filters I use. I like to play around with Photoshop a lot and learn new things through tutorials.

P.S.: The original .psd file is heavy so be patient >_> I added a resized version so the file is lighter. I use Photoshop CS4 and it's default brushes.
