You can see behind her that the room is a total mess. Ask about this (because maybe this is tied to the fact that Spike lived here before).

1: Scan. You'll need to know her emotional state for a while starting now, especially if she turns this into a prolonged discussion. 2: Confront. Not only is she deflecting, she's denying you and your sister before she's even hear you out. What's more, the room behind her aside, something else is wrong as well. Don't need to be a changeling to see those eyes. (I suspect she's been crying, confirm with emotions before suggesting this.) 3: Comfort. We would not be asking such a thing if we didn't want to help. Assure her that your only here to talk, but if everyone and their dog knows somethings wrong, she probably needs to talk a lot more than she thinks. Changelings are well know for their secrets, so if she's worried about what you might tell someone else, she needs only ask, and we won't tell a soul. (Flurry's pride & honor will keep her quiet.)

Take a page out of Applejack's book and be honest. Tell Twilight that it's obvious that her smile is fake and that you can tell she's been crying. Tell her that it's best if she talks with someone about the issue rather than avoid it. But agree to allow Twilight to talk only when she's ready. And if not with you, then with her friends, since by ignoring the issue, they just made it worse. However, to be sure that Twilight keeps her end of the deal and talks, you tell her that you won't leave her side until you talk to someone about the issue.

By the looks of it, it seems that she's having a hard time with something, probably related to Spike. Tell her not to hold the problem she has with Spike inside her, as that would be bad for everyone in the long run. Tell her to talk to someone about it; the longer she doesn't, the more she will beat herself up over it, leading to more problems.

It's obvious that something's wrong, and Twilight just deflected a direct inquiry. She may not want to talk about it, but her and Spike used to be so close, and it's always a shame to see something so beautiful tarnished. Also, you can see lines in her smile... she may not be sleeping well and Equestria needs it's Princess of *FRIENDSHIP* (One of the most powerful forces known to ponykind) in top condition. If she won't talk to us about it, she needs to talk to somebody. Preferably AJ or Spike himself. ACTION: Press forward gently, stating that she needs to talk to someone, and we want to listen. We want to help. "I don't like seeing my dear Aunt in pain. I want to help."

as a side note,, Vavacung, if she's naturally at 12 hp now and the scarf originally gave a +2 hp, shouldn't we have a maximum of 14? or was the full enchantment never restored?
