The Karstarks aren’t even the third greatest house in the north they only are the closest allies to the Starks. The richest and probably third greatest house are the Manderlys who are the only house in the North praying to the seven instead of the old gods. The fourth largest house are either the Karstarks or the Umbers. You met Greatjon Umber who passed during the Red Wedding he was the one who called out Robb as King (and Greywind ate his fingers during a banquet at winterfell)

I think it goes like this in the north: 1- House Stark 2- House Bolton 3- House Umber 4- House Glover 5- House Karstark House Karstark was founded by a Stark many years ago so it's basically the same family and the same root as the Starks but with a different name.
