Don’t know if you figured this out later or if anyone has already commented but Batman saved Dent because the Joker switched up their addresses on purpose, he was still trying to save Rachel which is why Gordon ends up at her place.

Batman went to the location joker told him Rachel was, the trick was he switched the locations because he knew who he would choose

Used this film as my case study for practical effects. Christopher Nolan blew up an actual building for the hospital scene, and the truck flip was all real. He put a lot of money into a hydraulic system inside the truck to flip it. He very rarely uses CGI when there's a way to film it for real.

As for Scarecrow not being in the movie much, if you remember the beginning in the parking structure, Batman caught him along with the fake Batman’s and handcufffed them to the railing in the parking garage. So most likely he got transported to the asylum
